Our office hours are Monday - Friday, 7:30am - 5:00pm.Make an Appointment

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.
What to Bring With You for Your Visit
- Medications in their original containers
- Any test results that are obtained from the referring MD. Some examples would be:
- EKG
- Holter monitor
- Stress test results
- Letters
- Past medical histories
- Any allergies to medications
- Insurance cards
- Referral for treatment from the primary care MD
New Patients
New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, medication bottles for all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
Billing
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.
Insurance
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We participate with most insurance plans. For your convenience you may review the complete list of insurance plans that we participate with. We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us promptly if you have any change in your name, phone number, address, or insurance. Please be prepared to present your insurance card at every visit.
Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we attempt to notify patients beforehand. If you are 15 minutes past you scheduled appointment time we will need to reschedule. Multiple no-shows or cancellations will result in dismissal from the practice.
Referrals
Please allow 48 hours for referral processing. If you are being referred, please be sure to bring the referral with you at the time of office check-in.
Some insurances may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.
If necessary, we will obtain authorization for testing scheduled by our office.
After Hours Emergencies
If you are experiencing a medical emergency, dial 911. However, if you need after hours advice for a medical concern, please call (615) 515-1900 and our answering service will assist you.
